The prevalence of diabetes is increasing at an alarming rate, and data show glycemic control is declining among those with type 2 diabetes. Most patients with type 2 diabetes receive diabetes care through their primary care provider, and despite advancements in glucose monitoring, only a small percentage use continuous glucose monitoring (CGM). Major limitations to improving glucose control are low clinician knowledge, skills, and confidence implementing CGM in the clinical setting. This expert panel will offer practical strategies for integrating CGM while enhancing provider knowledge, skills, and confidence for CGM implementation. The panel will answer audience questions and provide insight for best practices and addressing social determinants of health. By the end of the session, participants will leave with the necessary information and competence to integrate these novel strategies for improving diabetes control.
